USE NETCHEX TO REQUEST PTO. DO NOT USE THE PTO FORM!
Exception: At Year End, Netchex won't have your future Year's PTO loaded until after the last payroll processes. Use this form only then and use Netchex when your new balance gets loaded into Netchex.
